Filing a grievance

You may also file a civil-rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, electronically at the Office for Civil Rights Complaint Portal (ocrportal.hhs.gov), or by mail or phone.

How to file a civil-rights complaint
If you believe Heartway Corporation has failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability or sex, you can file a grievance with the administrator of your community in person, by mail or by phone — contact information for every community is on our contact page. If you need help filing a grievance, your community’s administrator is available to help you. Relay users may dial 711.
