Care that grows with your needs
From short-term rehabilitation to long-term skilled nursing, every Heartway community offers a full continuum of care — so residents can stay in the place they know as their needs change.

Around-the-clock licensed nursing
Skilled nursing is the heart of every Heartway nursing home. Licensed nurses are on site 24 hours a day, delivering physician-directed care — from medication management and wound care to IV therapy and post-hospital monitoring. Every resident’s care plan is built with their physician and reviewed as needs change. Skilled nursing stays are typically covered by Medicare (following a qualifying hospital stay), Medicaid, VA benefits or private insurance — our admissions team will walk your family through exactly what applies before you commit to anything.

Regaining strength and independence
After surgery, a fall, a stroke or an illness, the goal is simple: get you safely back to the life you know. Our in-house therapy teams provide physical, occupational and speech therapy right inside each community, so there’s no traveling to appointments while you recover. Short-term rehabilitation in a skilled nursing setting is one of the most common reasons families first come to a Heartway nursing home — and Medicare Part A typically covers qualifying short-term rehab stays.

- Speech & swallowing therapy
- Post-surgical & joint-replacement recovery

A stable, supportive home
When living at home is no longer safe or practical, long-term care offers something better than getting by: a true home, with neighbors, activities, home-style meals and a care team that knows every resident by name. Residents receive help with daily living — bathing, dressing, mobility, meals — while staying as independent as they’re able. Long-term care in our communities is most often funded through Oklahoma Medicaid, long-term-care insurance, VA benefits or private pay, and our team helps families plan the transition between payers ahead of time.
